First Visit
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on a separate day.
Please assist us by providing the following information at the time of your consultation: 1)Prior Dental record and X-rays (if applicable),
2) A complete list of your current medications. Please alert the office if you have a medical condition that may be of concern prior to your dental appointment.
3) If you have dental insurance, please bring card at your dental visit.
4) Please fill up the forms provided here, prior to your dental appointment.
IMPORTANT: A parent or guardian must accompany all patients under the age of 18 at the consultation visit.
